Have you ever visited us and wondered why we can’t see all of your health and care information? Or queried why you sometimes need to answer the same questions about your medications, allergies or treatment each time you visit your local GP or hospital?

The simple answer is we all use different computer systems to record your details. However, that’s now changing with the introduction of the Coventry and Warwickshire Integrated Care Record (ICR).

The ICR is bringing together the various electronic records of a person’s care and presenting it in a structured and easy-to-read format for health and care professionals to use.

What is an integrated care record?

An integrated care record (ICR) is a way of bringing together the various electronic records of a persons care. It takes information directly from the existing systems used by health and social care organisations and presents it in a structured, easy-to-read format for health and carte professionals.

What data will be available and who will be able to see it?

The first phase of the ICR will allow health and care professionals to view:

  • Your GP practice medical record
  • Secondary care and mental health data

Who can see my data through the Coventry & Warwickshire Integrated Care Record?

  • GP practices within Coventry and Warwickshire
  • Coventry and Warwickshire Partnership NHS Trust
  • University Hospitals Coventry and Warwickshire NHS Trust
  • George Eliot Hospital NHS Trust
  • South Warwickshire NHS Foundation Trust
  • Coventry City Council
  • Warwickshire County Council
  • West Midlands Ambulance Service University NHS Foundation Trust
  • Health and care organisations in the neighbouring areas of Birmingham and Solihull and Herefordshire and Worcestershire will be able to view your information for the purpose of giving you direct care should it be necessary.

What do I need to do?

You don’t need to do anything. For anyone who is registered with a GP in Coventry and Warwickshire, the change is taking place automatically. As the ICR now have the capability to include the details of people under the age of 18, their records will also be available for health and care professionals to view through the Shared Care Record.

What if I do not want my records to be available through the ICR?

You have the right to object at any time if you are 16 years of age or older. For anyone below the age of 16 your right to object form must be signed on your behalf by someone with parental responsibility. The ICR does not recommend objecting, as information that could be vital when you need health or social care support – for instance, during a visit to a hospital Emergency Department – might not be immediately to hand as a result.
Objecting will mean the services giving you care will be unable to view your records from other services.
