If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. We require proof of identity (passport, photo drivers license or valid ID card) which must state date of birth and proof of address (a current utility bill, HMRC Letter or council tax bill) dated within the past 3 months.
When patients apply to register with the practice, the practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability, or medical condition.
